A two-page DAP progress note template with select-and-fill sentences you can copy & paste straight into your EHR.
Want to spend less time & energy on notes? This is the simple template I use for sessions. Pick from the options, fill in a few sentences, & paste it into your EHR or note-keeping software.
What's inside
Data: session details, client presentation (2 options), risk (2 options), session focus (27 options), interventions (15 options) & 3 fill-in sentences
Assessment: 13 person-centered growth domains (easy to swap for your own) plus a progress & evidence line
Plan: client plan (5 options), changes, safety planning, between-session focus, next session & signature
A note on the assessment section: I use person-centered growth domains (rooted in Carl Rogers' work) to track client progress instead of behavioral goals. If that's not how you work, simply swap them for the domains, goals or treatment-plan objectives that fit your approach. Everything else works the same.
Best for: therapists who want faster, consistent progress notes
Format: 2-page PDF template · plain-text [ ] options you mark with an X · copy & paste friendly (no fillable fields)
