EMDR Brainstorm & Resourcing | Fillable EMDR Treatment Planning Template

$24.00

A fillable working template for EMDR history taking and preparation: the beliefs that resonate, the memories and triggers behind them, and the resources you build together to support the work.

It follows the flow I use with clients. Start with presenting concerns, goals & readiness. Have your client check the negative beliefs that resonate, then brainstorm the past memories, relationships & present-day triggers behind each one. Choose positive beliefs, build a three-pronged target sequence, and then turn what you learned into a resource plan, with a question bank for finding the gaps, wounds & unmet needs each belief points to and the resources that answer them.

Every section has fill-in boxes & checkboxes you can type & click directly into. Open it in Adobe Acrobat Reader (free), Apple Preview or most browsers. Keep a blank master and save a copy for each client.

What's inside

  • Clinician guide: where it fits in the protocol, how to use it & clinical notes

  • History taking: goals, readiness checklist, 111 belief checkboxes in theme cards, belief brainstorm, positive beliefs & target sequence with floatback

  • Resource brainstorming: a question bank by theme + resource plan

  • Resource installation: calm place, RDI, resource figures & container

  • Readiness: session tools, parts & a checklist before reprocessing

Best for: EMDR-trained therapists who want one organized place for history taking, target planning & resourcing

Format: 18-page fillable PDF · type-in boxes & clickable checkboxes · printable

For licensed mental health professionals trained in EMDR. This template isn't exhaustive and isn't a protocol: it's the collection of history-taking prompts and resourcing options I pull from, and not every section is meant for every client. It does not ensure the EMDR protocol is being followed correctly. Licensed for use with your own clients; not for resale or redistribution. All sales final.
